The Kentucky Department for Public Health reported the communicable disease reporting requirements for healthcare professionals and laboratories outlined in 902 KAR 2:020 have recently been amended, including some new conditions and mortality reporting, as well as changing reporting requirements for several conditions.
The “Reportable Disease Letter” details the exact changes for your awareness. The updated “Kentucky Reportable Diseases Conditions Amended Table” is provided as a helpful tool that can be distributed and posted where appropriate for use in facilities and by clinicians.
Here are updated fillable versions of Kentucky’s reportable disease forms, KDPH’s EPID series:
1) EPID 200 – Most reportable conditions are reported with this form
2) EPID 250 – For reporting of multi-drug-resistant organisms (MDRO’s)
3) EPID 394 – Hepatitis Infection in pregnant women or young children
4) EPID 399 – Perinatal HBV prevention form for exposed infants and HBV-positive pregnant women
Please replace any existing versions of EPID forms with these updated version when reporting.
